In order to submit a lost animal report, you must adhere to the following:

You MUST be a Jessamine County resident. We will not regard lost reports filed for animals living outside of our jurisdiction.
You must be the immediate owner or guardian of the animal. We cannot accept applications filed by non-owners of the animals.
You are responsible for contacting us via email or phone if your animal returns home so that we may update our lost report.

Was the animal wearing a collar or any type of identification when it went missing? (Ex. red cloth collar, brown leather collar, grey tick collar, orange bandana, etc.)(Required)

What is the exact date and the most accurate time that you believe the animal went missing? (Ex. 10-01-2018) DO NOT reply with "yesterday", "last week", etc.)(Required)

What is the area that the animal went missing from? (Ex. My house at 120 Fairgrounds Way, Nicholasville, KY, 40356. The neighborhood of Beauford Place, Nicholasville, KY, 40356.
